About Us

Mom Starts Here is a nonprofit dedicated to walking alongside moms through pregnancy, parenting, and beyond. We believe every mother deserves support, dignity, and material support. We make that happen through practical care, community-based support, and deep relationships. Our programs include a Baby Item Network, support groups, parenting education, maternal health navigation, and more.

Through our Maternal Health Program, we offer home-based outreach, health screenings, and comprehensive support to pregnant and postpartum women in collaboration with local healthcare providers and community organizations.

About the Role

We are seeking a Part-Time Maternal Care Community Health Worker to serve pregnant and postpartum women in Schenectady County through home visits and outreach. This is a flexible, weekday (daytime) field-based role perfect for someone with a heart for service, strong communication skills, and a desire to make a meaningful impact in maternal health.

Responsibilities

  • Conduct home visits and outreach with expectant mothers
  • Provide education on prenatal wellness, postpartum care, and infant care
  • Collect health vitals during home visits and share with healthcare providers
  • Help clients navigate healthcare and services, access community resources
  • Complete client intake, assessments, and documentation
  • Connect families to Mom Starts Here services, like baby supplies and support groups
  • Collaborate with partner agencies and attend team meetings

What We’re Looking For

  • Experience working with vulnerable or underserved populations
  • Passion for or experience in maternal and prenatal health
  • Strong written and verbal communication skills
  • Self-starter who can manage a flexible schedule
  • Reliable transportation and willingness to travel within Schenectady County
  • High school diploma or equivalent required; degree in human services, public health, or related field preferred
  • Bilingual (especially Spanish-speaking) candidates are encouraged to apply
