- Obtains a complete patient past medical history and history of present condition.
- Conducts a comprehensive system review of the patient.
- Completes accurate and appropriate tests and measures.
- Determines the appropriateness of physical therapy treatment following examination.
- Makes referrals to other practitioners as needed.
- Determines the physical therapy diagnosis based on examination results.
- Establishes a plan of care specifying interventions, timing, and frequency.
- Reexamines patients to determine changes in status and modify or redirect interventions accordingly.
- Uses various physical therapy procedures and techniques to produce desired outcomes.
- Evaluates treatment effects and modifies the plan of care as needed.
- Provides safe and effective patient care, reducing the likelihood of medical errors.
- Performs patient education in a timely manner to prevent delays in discharge.
- Treats patients and families with respect and dignity.
- Coordinates patient care to ensure needs are met and hospital policies are followed.
- Determines anticipated goals and expected outcomes for each episode of care.
- Participates in outcomes data collection and analysis to assess intervention effectiveness.
- Maintains accurate and timely documentation meeting current standards and policies.
- Interacts professionally with patients, families, and involves them in the formation of the plan of care.
- Manages patient schedules accurately.
- Collaborates with other departments as necessary for patient care and performance improvement activities.
- Graduate of an accredited University or College of physical therapy.
- License or temporary permit from the State of New York.
