Summary
Position Highlights:
- Provides social work services and patient navigation for and in partnership with patients, families, and significant others. These include concrete service provision; implementation of care plans as it relates to transitions of care and patient navigation.
- Makes referrals to outside agencies and assists in coordination of continuity of care.
- Works collaboratively with and provides leadership within the multidisciplinary team to optimize patient outcomes.
- Shall be highly knowledgeable about community resources, continuity of care coordination, and all aspects of the discharge planning process.
- Will seek mentorship for professional growth and development.
- The Social Worker represents and supports the mission, philosophy and goals of the Cancer Center.
The Ideal Candidate
The ideal candidate will have the following qualifications:
- BSW from CSWE approved school
- One-year experience in social work or internship in medical or psychiatric social work preferred
- Experience in oncology and/or human service agencies
Responsibilities:
- Anticipate and coordinate referrals to community-based organizations, to ensure the timely continuation of a patient’s treatment plan or discharge following an acute care admission.
- Collaborate effectively with medical team.
- Complete all documentation and reporting requirements.
Credentials and Qualifications:
- BSW from CSWE approved school; bachelor's in psychology, counseling, or behavioral health science
- One-year experience in social work or internship in medical or psychiatric social work preferred
- Must have clear written and verbal communication skills and basic competence in various computer applications
