Responsibilities:

• Provide high-quality care and support to clients in their homes, focusing on their physical, emotional, and social needs
• Assist with daily living activities such as bathing, dressing, grooming, and toileting
• Assist with mobility and transfers, including walkers, canes, and wheelchairs
• Provide medication reminders and assistance with medication management
• Collaborate with our interdisciplinary care team to develop and implement individualized care plans
• Promote independence, dignity, and well-being of clients, while encouraging social interaction and engagement

About the Role:

As a Home Health Caregiver with Modani Care, you will work closely with our team of healthcare professionals, including registered nurses, therapists, and social workers, to ensure that clients receive comprehensive and coordinated care. You will have the opportunity to build meaningful relationships with our clients and their families, and make a positive impact on their lives.
