- Provide personalized care and support to clients in their homes, assisting with daily living activities such as bathing, dressing, grooming, and medication management
- Develop and implement individualized care plans with clients, families, and healthcare teams to ensure continuity of care
- Assist with meal preparation, light housekeeping, and laundry as needed
- Monitor and report changes in client condition, medication side effects, and other relevant health information
- Maintain accurate and detailed records of care provided, client progress, and any concerns or issues that arise
- Collaborate with other healthcare professionals, including physicians, registered nurses, and occupational therapists, to ensure seamless care coordination
- Demonstrate knowledge and understanding of state-specific programs and regulations governing home care services
