Job Summary:
The Social Services Assistant will work closely with our multidisciplinary team to identify and address the social, emotional, and practical needs of our patients. This includes assessing patient needs, developing and implementing individualized plans, and connecting patients with community resources. Strong communication and interpersonal skills are essential for success in this role.
Responsibilities:
Conduct social and emotional assessments of patients to identify their needs and develop individualized plans
Collaborate with healthcare team members to ensure comprehensive care planning
Connect patients with community resources, including government benefits, housing, and transportation services
Provide emotional support and counseling to patients and their families
Maintain accurate and detailed records of patient interactions and progress
Participate in patient care conferences and care planning meetings
Requirements:
High School Diploma or equivalent required
Strong communication, interpersonal, and organizational skills
Ability to work effectively in a fast-paced environment
Familiarity with community resources and services
