Responsibilities:
- Conduct social and emotional assessments of patients to identify their needs and develop individualized plans
- Collaborate with healthcare team members to ensure comprehensive care planning
- Connect patients with community resources, including government benefits, housing, and transportation services
- Provide emotional support and counseling to patients and their families
- Maintain accurate and detailed records of patient interactions and progress
- Participate in patient care conferences and care planning meetings
Requirements:
- High School Diploma or equivalent required
