About the RoleYou will treat seniors in:
- Private homes
- Assisted living
- Memory care communities
- Residential care facilities
Your mission: help seniors stay safe, confident, and independent where they live.
This is an outpatient role (not home health). Documentation is completed with the patient — no notes taken home.

Requirements- Active California Physical Therapist License
