- 13 Week Duration | Days | 4x9-hour shifts | 36 hrs
Travel Offer:
- Estimated Weekly Total: $2,700.40
- Stipend included: $1,246
- Hourly Pay: 40.40, O.T: $60.60
- Travel/Mileage reimbursement: $500

JOB DESCRIPTION
- Primary Location: Tuba City Regional
- Shift Requirements: Monday to Friday, 8am-5pm
- Float Requirements: N/A
- On Call/Call Back Requirements: Weekend on call rotation, would be 1 weekend a month
- Weekend Requirements: Would be On Call 1 weekend in a month
- Holidays: As needed
- Unit Information: Social Services

SUBMISSION REQUIREMENTS
- Must have a minimum of three (3) years of experience in the clinical care setting - Required
- Experience and knowledge of discharge planning, case management, or utilization review processes.
- Demonstrate knowledge of electronic health record systems.
- Master’s Degree in Social Work (MSW)
CERTIFICATION REQUIREMENTS
- BLS (AHA)
