Department: Population Health - Care Management Support Services Shift: Primarily Days (United States of America) Employee Type: Regular Per Diem Type (if applicable): Minimum Pay Range: $26.10 - $32.62 Job Description: POSITION SUMMARY Proactively navigates patients within the health system to support high quality outcomes, ease of access to care, and excellent patient experiences. Collaborates across departments to ensure patients are engaged in health promotion and chronic condition management activities as well as advocate for individual needs. Educates regarding and promotes Marshall resources and programs to support patient-centric holistic care and quality outcomes. Utilizes reports and data to longitudinally monitor patient progress and needs as well as identify opportunities for improvement. Continuously works to elevate the Marshall patient experience with role model level customer service. POSITION QUALIFICATIONS Education/Licensure/Certification: Bachelor’s degree in health-related field, preferred. Medical Assistant experience, preferred. Knowledge/Experience: Minimum 2 years of experience in customer service or healthcare related field Knowledge of basic healthcare system structure and health insurance, required. Medical office operations knowledge or medical assistant experience, preferred. Epic EMR knowledge, preferred. Skills: Exemplary customer service skills, required. Excellent organizational and interpersonal skills, required. Intermediate ability with Word and Excel software, required. Excellent time management skills, required.
