The Peer Recovery will be responsible for intake coordination, client enrollment, assessment of service needs, treatment plan development, and navigation of recovery support services. Key responsibilities include:
- Conduct comprehensive intake assessments to determine client eligibility, immediate needs, and appropriate service pathways.
- Develop individualized treatment and recovery service plans in collaboration with clients and the multidisciplinary team.
- Monitor client progress toward service goals through regular checkins, reassessments, and plan updates as necessary.
- Coordinate referrals and service linkages to communitybased providers, including outpatient treatment programs, housing services, employment supports, mental health services, and primary care.
- Maintain accurate, timely, and confidential documentation of all client interactions, service activities, referrals, and outcomes in accordance with program standards and agency policies.
- Support clients in overcoming barriers to accessing services, advocating for client needs within the community system of care.
- Collaborate closely with Peer Recovery Support Staff to ensure seamless client engagement, service continuity, and holistic care planning.
- Participate in multidisciplinary team meetings, case consultations, and staff trainings as required.
- Operate under the supervision of the Clinical Director to ensure compliance with all licensing, grant, and programmatic requirements.
- Uphold traumainformed, culturally responsive, and clientcentered care practices throughout service delivery.
