The Hospice Care Planner coordinates and develops care plans for patients requiring hospice services, ensuring smooth transitions from hospital discharge to appropriate hospice programs.
This role supports business development by maintaining relationships with physicians, social workers, and case managers within a major medical center.
Our Hospice and Palliative Care programs offer comprehensive support to help patients and their families manage life-limiting illnesses or end-of-life care.
As a key member of the business development team, the Hospice Care Planner develops and maintains positive relationships with healthcare professionals and families.
The Hospice Care Planner will evaluate and establish a plan of care for patients referred for Hospice Care services, facilitating admission to a suitable Hospice program.
Responsibilities:
- Licensed Registered Professional Nurse in NYS with current Registration required.
- Bachelor's Degree preferred.
- Minimum two years experience in Hospice nursing with proficiency in physical assessment and interviewing skills.
- Client-service focused with excellent written and verbal communication skills.
- Advanced critical thinking and problem solving skills, plus ability to work independently essential.
- Bilingual Spanish/English may be required in some locations.
Salary : $90,000 - $118,000
