Job Responsibilities
- :In this position, you will provide psychosocial support and Social Work services to Hospice patients and their family members who are at an inpatient unit, nursing home or home care settin
- gYou will provide pre and post bereavement service
- sYou will serve as a social work member of hospice IDG and as liaison between hospice, inpatient and nursing home staf
- fYou will develop the social work plan of care, set goals and make recommendations based on the patient and family need
- sYou will maintain accurate and current documentation reflecting palliative social work assessments an

Requirement
- s:Graduate from the an accredited School of social wo
- rkMinimum of one year experience in Social Work in a healthcare setti
- ngKnowledge of Word and excel preferr
- edLicensed Master Social Worker (LMSW or LCSW) required in the state of New Yo
- rkValid Driver license and car requir
- edAbility to work within an interdisciplinary team environme
- ntPossess excellent organizational, communication and listening skil
- lsAbility to input clinical data proficient

Salary : $68,000 - $80,000
