About the Role
MJHS Hospice and Palliative Care is seeking a compassionate Registered Nurse for a field-based Hospice Case Manager position. In this role, you will conduct physical and psychosocial assessments for patients within community and facility settings. You will be responsible for developing, implementing, and managing individualized nursing plans of care while utilizing electronic documentation to record patient visits.
Key Responsibilities
- Conduct comprehensive physical and psychosocial assessments during field visits.
- Develop, implement, and adjust patient-centered nursing plans of care based on ongoing needs.
- Electronically document all patient visit occurrences accurately and timely.
- Provide compassionate end-of-life and palliative care across various community and facility environments.
Requirements
- Associate’s Degree or Diploma from an accredited school of Nursing (BSN preferred).
- Active New York State (NYS) RN License.
- Valid New York State (NYS) Driver’s License.
- 2 years of experience in medical-surgical (med/surg), community health, or oncology nursing is highly preferred.
