- This contract position, located in Jamaica Estates, NY, requires working onsite from Monday to Friday for a total of 40 hours each week.
- Responsibilities include providing comprehensive care, managing patient treatment plans, and collaborating with healthcare professionals.
- Ability to develop care plans based on patient needs and team skills.
- Proficiency in implementing organizational programs and procedures.
- Experience in data collection and process adjustment for program requirements.
