What are the responsibilities and job description for the Case Manager for Summit Rehab position at Mitchell Hollingsworth Nursing and Rehabilitation?
Mitchell-Hollingsworth Nursing and Rehabilitation Center, LLC is looking for a Case Manager for Summit Rehab. The hours are 8:00 a.m. until 4:30 p.m. Case Managers are responsible for knowing all patients and sponsors on their assigned unit. This includes any issues and concerns through identifying and monitoring their spiritual, social, psychosocial, emotional, and recreational needs, etc. Coordinates the discharge process with the Medical Director, Nursing, and Therapy Departments. Must have good time management skills, communication skills, and be detail oriented. Must have good computer skills or Social Work Designee or Degree preferred,
Job Duties are (but not limited to):
- Coordinate family care plan meetings with therapy department
- Investigate patient/family complaints, concerns, grievances, and problems. Report to Resident Services Director and appropriate other Managers.
- Arrange transportation to and from patient appointments
- Meet with patient representatives to assist them with resident needs, i.e. financial, insurance, support agencies, etc.
- Be able to learn/or have knowledge of patient's insurance for skilled rehabilitation. and community services.
- Guide the patient/family through discharge planning from rehab into the community or into long term care, including home health, hospice, medical equipment etc.
- Document daily tasks
- Complete any required assessments
- Other duties as assigned by Senior Case Manager, Resident Services Director or Administration
