Position Summary
Mission Valley Post Acute is seeking a compassionate and detail-oriented Case Manager to join our team on a part-time basis. The Case Manager plays a critical role in coordinating resident care, facilitating communication between interdisciplinary teams, and ensuring smooth transitions throughout the continuum of care.
Key Responsibilities
- Coordinate and manage resident care plans in collaboration with nursing, therapy, and social services teams
- Monitor resident progress and adjust care plans as needed
- Facilitate discharge planning, including coordination with families, physicians, and outside agencies
- Ensure compliance with all regulatory and documentation requirements
- Maintain accurate and timely case management records
- Act as a liaison between residents, families, and healthcare providers
- Assist with insurance verification and authorization processes when applicable
- Identify and address barriers to effective care and safe discharge
- Participate in care conferences and interdisciplinary meetings
Qualifications
- Previous experience in case management, healthcare coordination, or social services preferred
- Knowledge of post-acute care processes and regulations is a plus
- Strong organizational and multitasking skills
- Excellent communication and interpersonal abilities
- Ability to work independently and collaboratively within a team
- Proficiency in basic computer systems and documentation
