What are the responsibilities and job description for the MEMORY CARE COORDINATOR position at Mission Senior Living?
- Reviews and documents daily in the communication log as appropriate.
- Reviews new orders and EMAR daily.
- Supervises and reviews the transcription process, medication and equipment ordering, pharmacy cycle fill.
- Performs resident care coordination and communication with outside agencies and medical providers.
- Reviews alert charting.
- Reviews incident reports as needed.
- Reviews cleanliness and organization of medication room.
- Reviews, supervises and assists with resident care as needed.
- Performs medication pass.
- Assists residents ADLs.
- Reviews and documents on the service plan as needed.
- Attend service plans review as needed.
- Reviews and responds to Community IQR audits to ensure compliance.
- Assist and/or conduct the orientation for new team members.
- Schedule the exit-training interview, with new team members and review of skill checklist.
- Performs all other duties as assigned by any member of leadership.
Qualifications:
- Keep all required licensure in good standing at all times.
Physical Demands:
- Ability to push, pull, and lift at least 50lbs.
- Ability to bend, squat, and climb stairs.
- Ability to continuously walk or stand.

Salary : $55,000 - $62,000
