Position Classification: Exempt Salary: $64,480 - $80,000.
Position Status: Full-Time Regular.
Employee Type: Supervisory.
Estimated Weekly HRS: 40.
Reports To: Clinical Director.
Job Purpose & Summary:
This role is responsible for the successful implementation, coordination, and management of our unique specialized clinic designed to serve our chronically ill patients. This role is accountable to our organization's mission and vision while overseeing the integration of services. Additionally, this role will also be responsible for supporting and coordinating the healthcare of patients.
Job Tasks:

Program Integration:
  • Collaborate with various team members on special programs to ensure the seamless integration of the ECM Program.
  • Work closely with cross-functional teams to establish and achieve program goals.

Staffing and Team Development:
  • Manage and administer clinical staff in areas of performance, patient care, conduct, and behavior etc.
  • Collaborate with HR to recruit, hire, and onboard staff, ensuring alignment with the program's objectives.
  • Serve as a coach/mentor to assist in the development of staff and continuing education.

Patient Engagement and Commitment:
  • Implement and manage the Patient Compact to secure commitment from patients and their families.
  • Administer and enforce program contractual obligations to secure and maintain commitments from patients and their families.
  • Work across multiple departments to ensure effective and efficient workflow for the continuity of healthcare.

Care Coordinator:
  • Provide Patient education for chronic disease management.
  • Provide additional resources to support the patient in aspects of financial, social, and community.
  • Conduct needs assessment for additional services and follow-up care to include but not limited to: providing support and resolution to additional services beyond the clinic.
  • Assist patients with referral inquiries.
  • Manage the patient population to ensure preventative measures are performed within the published periodicity timelines.
  • Gather and compile patient data to gauge and track treatment response for changes in medication adherence and access to care.
  • Create a care plan by collaborating with providers.
  • Track and record patient follow up and clinical outcomes.
  • Document telephone encounters in the care plan.
  • Use patient database to identify potential patients who may be a great fit for the specialty clinic.
  • Accurately enter and document patient related data within the EMR system.

Facility Management:
  • Review, research, and make recommendations regarding the allocation and utilization of company resources.

Financial Oversight:
  • Collaborate with the finance department to manage the budget for the ECM Program.
  • Monitor revenue generation, ensuring the program meets financial targets and sustainability, propose recommendations or revisions.

Long-Term Planning:
  • Propose, contribute, and participate to the development of the clinic and/or its programs.
  • Performs other duties as assigned.

Knowledge/Characteristics:
  • Proven experience in program management within a healthcare setting, preferably in a managerial capacity.
  • Strong understanding of public health principles and their application in program development.
  • Excellent communication, leadership, and interpersonal skills.
  • Ability to collaborate effectively with diverse stakeholders, including medical professionals, administrative staff, and external partners.
  • Demonstrated ability to manage budgets and financial resources effectively.
  • Familiarity with healthcare regulations and compliance standards.

  • Minimum Education: Associate’s degree.
  • Preferred Education: Bachelor’s Degree.
  • Minimum Years of Experience: 1-3 years.
  • Preferred Years of Experience: 5 years.
