We are seeking an on-call Nurse Practitioner to work in our community clinics serving the vibrant and diverse Mission and Excelsior districts and surrounding region.
This provider will cover days and shifts during time-off of regular providers in clinics performing primary care as well as seeing walk-in/urgent care patients according to schedule availability. This position provides tremendous flexibility of schedule, as the provider has freedom to accept or pass on available days in which regular providers have planned time-off. The provider may also receive inquiries of next-day or same-day availability in the case of regular providers out due to illness. The work will include a mix of general adult and women’s care, with the possibility of including some Pediatrics as well, according to the provider’s preference and level of experience. The provider will serve the patients as a member of an integrated team including MAs, Nurses, Behavioral Health, Dieticians, etc.
Essential Functions & Responsibilities
The on-call Nurse Practitioner has the following responsibilities and duties:
- Provides ambulatory acute, chronic, and preventative medical services in accordance with national standards of practice.
- Provides continuity of care for hospitalized and referred patients according to Health Center protocols with appropriate consultation of patients with senior medical staff.
- Meets credentialing requirements of managed care plans contracted by MNHC.
- Completes necessary credentialing and re-credentialing applications.
- Participates in Quality Assurance initiatives as designated by the Clinical Coordinator.
- Other related duties within scope of practice as assigned.
Qualifications
The on-call Nurse Practitioner should have the following skills, education and experience:
- Current and valid NP license and furnishing license (FNP or ANP) and DEA license.
- Experience in a Community Health Center or comparable work preferred.
- Bilingual in Spanish and English required (populations served in clinics are 60-80% Spanish monolingual).
- Board Certified or Board Eligible.
- Current BLS.
