Position: Care Coordinator
Reports To: Empowerment Center Lead; Day Services Director; Program Services Director; Executive Director
Position Location: Polson, MT
Classification: Full-Time (40 hours/week) with occasional overtime as necessary
Hours: Monday–Friday, 8:00 a.m.–4:00 p.m.
Wage: $18.50–$19.50 per hour, DOE
Job Summary
The Care Coordinator ensures appropriate and meaningful interventions and procedures are followed to support quality care for individuals served by Mission Mountain Empowerment (MME). This position participates as a member of a multidisciplinary team providing relevant services and interventions. The Care Coordinator is responsible for supporting day-to-day client care needs from both an administrative and direct-care standpoint and serves as a key member of the Empowerment Center leadership team.
Duties and Responsibilities

Safely transport clients to and from appointments as needed or requested.

Coordinate transportation for clients and staff to and from appointments.

Complete client care checks, including but not limited to check-and-change documentation, medication checks, and weight checks.

Maintain acceptable attendance to ensure consistent individual care.

Ensure the health and safety of clients and staff at all times.

Communicate daily with the Empowerment Center Lead regarding logistics and transportation concerns that may impact client appointments.

Maintain thorough, consistent communication with the Lead and Assistant Lead to support decision-making and coordinate daily needs.

Ensure staff and Shift Leads at group homes receive timely instructions and information regarding client appointments and expectations.

Ensure all required supplies are prepared prior to appointments, including medications, MARs, Medical Report Forms (MRFs), clothing changes, and incontinence supplies.

Assist with transporting and accompanying clients to medical, dental, and health-related appointments.

Ensure MME medication administration policies are followed accurately and consistently.

Ensure all protocols are followed as written.

Ensure the special nutritional needs of individuals are followed as outlined in care plans.

In the absence of the Lead or Assistant Lead, communicate directly with the Day Services Director or Program Services Director.

Monitor the Empowerment Center's supply inventory and assist with ordering when supplies are low.

Perform other duties as assigned by the Empowerment Center Lead, EC Director, Program Director, or Executive Director.
Qualifications

High School Diploma or GED required.

Associate’s or Bachelor’s degree preferred from an accredited college or university in Human Services, Healthcare, Psychology, Education, or a related field.

Minimum of one (1) year of experience working in a related field.

Other education, experience, training, licensure, or certification as required by state or regulatory standards.

Ability to obtain and maintain Medication Certification, CPR, First Aid, and required behavioral trainings per MME standards.

Valid driver’s license and ability to meet MME driving requirements.

Proficiency with general computer applications, including but not limited to Microsoft Word, Excel, Google Workspace, and PowerPoint.

Public Relations
Represents Mission Mountain Empowerment in the community as delegated by the Executive Director.
Salary : $19 - $20
