Job Summary and QualificationsThe Discharge Panner coordinates patients’ transition from the facility to next level care, ensuring a safe and timely discharge. They assess patient needs, collaborate with physicians, nurses, families and community providers, arrange post-discharge services, verify insurance coverage, and educate patients on their discharge plan to support continuity of care and reduce hospital readmissions.
What qualifications you will need:- Nonviolent Crisis Intervention must be obtained within 30 days of employment start date
- Notary Public must be obtained within 6 months of employment start date
- Associate Degree, or Bachelors Degree must be obtained within 30 days of employment start date
