Palliative Nurse Case Manager

Mission Hospice & Home Care serves patients and families in the San Francisco Peninsula and South Bay with quality care and compassionate end-of-life support and education. Founded in 1979, we are a local, independent nonprofit that has provided thousands of patients and their families with physical, emotional, and spiritual support throughout the journey from a life-threatening diagnosis through death and bereavement.

The Palliative Nurse Case Manager will work as a team member with an assigned caseload, provide skilled nursing assessment, plan and care to patients and their families with chronic diseases

Mission Hospice and Home Care follow all CDPH vaccine requirements for healthcare personnel.

Essential Duties & Responsibilities include (but are not limited to):

• Assume primary responsibility for a patient/family using the nursing process to assess, plan, implement and evaluate patient/family needs, goals, and interventions
• Coordinate all patient/family services and prioritize needs with the members of the interdisciplinary team (IDT)
• Use a case management approach and refer to other services as needed
• Coordinate the implementation of the plan of care and ensure care coordination for each patient
• Provide palliative nursing evaluations to patients and their families including assessment, planning, goals, and interventions
• Seek input from other team members regarding patient plan of care to obtain additional knowledge and support.
  
  

Education/Experience

• One year experience as a professional nurse in the past three years or Bachelor’s degree in Nursing from an accredited School of Nursing
• Minimum of two years of professional nursing experience with at least two years in an acute care, home health or palliative care/hospice setting.
  
  

Certificates, Licenses, Registrations

• Current license as a registered professional nurse in the State of California
• HPNA certification preferred
  
  

Pay Range

$52.90 - $66.20 hourly
