$52.90 - $66.20 hourly

Essential Duties & Responsibilities include (but are not limited to):
- Assume primary responsibility for a patient/family using the nursing process to assess, plan, implement and evaluate patient/family needs, goals, and interventions
- Coordinate all patient/family services and prioritize needs with the members of the interdisciplinary team (IDT)
- Use a case management approach and refer to other services as needed
- Coordinate the implementation of the plan of care and ensure care coordination for each patient
- Provide palliative nursing evaluations to patients and their families including assessment, planning, goals, and interventions
- Seek input from other team members regarding patient plan of care to obtain additional knowledge and support.
- One year experience as a professional nurse in the past three years or Bachelor’s degree in Nursing from an accredited School of Nursing
- Minimum of two years of professional nursing experience with at least two years in an acute care, home health or palliative care/hospice setting.
- Current license as a registered professional nurse in the State of California
- HPNA certification preferred
$52.90 - $66.20 hourly
