Hospice Nurse Case Manager

Mission Hospice & Home Care serves patients and families in the San Francisco Peninsula and South Bay with quality care and compassionate end-of-life support and education. Founded in 1979, we are a local, independent nonprofit that has provided thousands of patients and their families with physical, emotional, and spiritual support throughout the journey from a life-threatening diagnosis through death and bereavement.

The Hospice Nurse Case Manager is responsible for the assigned caseload or facility where they will be providing skilled nursing assessment, planning and care to maximize the comfort and health of patients and families.

Tuesday, Wednesday, Thursday & Friday; 8:30am - 5:00pm

Service Area

San Jose with additional coverage to San Mateo as needed

Mission Hospice & Home Care follows all CDPH vaccine requirements for healthcare personnel.

Essential Duties & Responsibilities include (but are not limited to):

• Assume primary responsibility for a patient/family using the nursing process to assess, plan, implement and evaluate patient/family needs, goals, and interventions
• Coordinate all patient/family services and prioritization of needs with the members of the transdisciplinary team
• Use a case management approach and refer to other services as needed
• Coordinate the implementation of the plan of care and ensure care coordination and case management for each patient
• As applicable, apply specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and patient’s eligibility
  
  

Education/Experience

• One year experience as a professional nurse in the past three years or Bachelor’s degree in Nursing from an accredited School of Nursing
• Experience in home health and/or hospice setting preferred
  
  

Certificates, Licenses, Registrations

• Current license as a registered professional nurse in the State of California.
• Public Health Nursing certification highly desirable
• HPNA certification preferred
  
  

Pay Range

$58.00 - $73.65 hourly
