Mission Hospice & Home Care serves patients and families in the San Francisco Peninsula and South Bay with quality care and compassionate end-of-life support and education. Founded in 1979, we are a local, independent nonprofit that has provided thousands of patients and their families with physical, emotional, and spiritual support throughout the journey from a life-threatening diagnosis through death and bereavement.

We are currently seeking an RN to be a part of our team. The ideal candidate will provide coverage for After-Hours to cover patient/staffing needs and processes admissions for eligible hospice patients in coordination with Team Manager.

Schedule

Monday - Friday.; 4:00pm-12:30am.

Location:

Alameda County with additional coverage to San Mateo County as needed

Essential Duties & Responsibilities include (but are not limited to):

  • Make in-person nursing visits to patients as assigned, or as indicated.
  • Provide continuing care to patients/families as requested and in compliance with agency policies and procedures and applicable federal and state regulations.
  • Use the nursing process to assess, plan, intervene and evaluate following patient/family needs and goals.
  • Provide effective management of symptoms, including prompt escalation to providers or others as indicated.
  • Coordinate patient/family services and prioritization of needs with the members of the transdisciplinary team or outside providers/services.

Education/Experience

  • Associate's or Bachelor’s degree in nursing from an accredited School of Nursing
  • Experience in acute care/ home health and/or hospice setting preferred

Certificates, Licenses, Registrations

  • Current license as a registered nurse in the State of California.
  • Public Health Nursing certification highly desirable.
  • HPNA certification preferred.

Pay Range

$55.50 - $69.50 after hours/weekend shift differential
