Schedule/Shift: Part-time working Monday-Friday, 10am-2pm
Territory/Location: Ventura County
Responsibilities
- Reviews clinical documentation to help determine home health appropriateness.
- Assists in compiling and reporting on clinical metrics shared with community partners.
- Tracks and updates team members on new hospitalizations, hospital holds and resumptions of care.
- Must be self-driven with a high degree of attention to detail.
- Communicates with patients, families and caregivers regarding home health services.
- Coordinates with physicians and referral sources (as appropriate) regarding home health plans of care.
- Assists in ensuring that patients are seen in a timely manner and works with physicians to receive updated start of care orders when necessary.
- Cross trains to provide back-up coverage for scheduling and intake coordination when needed.
- Assist with field visits if needed.
- Uses equipment and supplies effectively and efficiently.
- Participates in personal and professional growth and development.
- Other tasks and responsibilities as assigned.
- Graduate of an accredited practical nurse or vocational nursing program.
- One (1) year nursing experience in home care, hospice care, institutional hospital or facility care preferred.
- Currently licensed as a licensed practical nurse or licensed vocational nurse in the state of service.
- Complies with accepted professional standards and practice.
- Demonstrates good verbal and written communication, and organization skills.
- Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.
- Possess and maintains current BLS CPR Certification.
