Title: Community Health Worker - Outreach, Enrollment, & ECM Support

Compensation : $21 / hour

Location: Inland Empire: Victorville, Barstow, Pomona, Corona

Schedule: M-F 8 to 4:30 PM Weekend Availability


Mission City Community Network (MCCN) is a Federally Qualified Health Center committed to providing high‑quality, culturally responsive care to low‑income and underserved families across the San Fernando Valley, San Gabriel Valley, Los Angeles, and San Bernardino regions. We offer comprehensive primary care services for children, adolescents, and adults.


We are seeking a Bilingual English/Spanish Community Health Outreach Worker to support our Victorville and Barstow communities through outreach, enrollment, and Enhanced Care Management (ECM)engagement.



Enhanced Care Management (ECM) Support


  • Engage hard‑to‑reach ECM members through field visits, phone outreach, and culturally sensitive communication.
  • Assist members in navigating the healthcare system and overcoming barriers to medical and behavioral health care.
  • Support RN Care Managers, Behavioral Health Care Managers, and Care Coordinators with delegated panel management tasks.
  • Participate in case reviews and collaborate with multidisciplinary care teams.
  • Monitor treatment adherence, including medication and shared care plan goals.
  • Track required assessments and screenings, including Comprehensive Health Assessments and Shared Care Plans.
  • Connect members to social services, community resources, and health education materials.


Outreach, Enrollment & Community Engagement


  • Conduct community outreach to identify individuals and families needing health coverage or care coordination support.
  • Screen patients for eligibility for programs such as Covered California, Family PACT, CHDP, and others.
  • Assist with health insurance enrollment, including application support, follow‑up calls, documentation uploads, and system updates.
  • Become (or be willing to become) a Certified Enrollment Counselor (CEC).
  • Provide culturally appropriate education on health topics, pandemic‑related issues, and available MCCN services.
  • Train patients to use digital tools such as patient portals, telehealth platforms, and mobile communication apps.
  • Build and maintain relationships with schools, community organizations, churches, parent groups, and other partners.
  • Develop outreach materials, presentations, and community education resources.
  • Conduct home visits or community‑based visits as needed to support outreach and enrollment goals.
  • Maintain accurate documentation and reporting in accordance with program requirements and HIPAA guidelines.
  • Accompanies ECM Members to office visits, as needed, and in the most easily accessible setting, within IEHP and healthcare organization guidelines; and any other visits as required


Patient Support & Administrative Duties

  • Interview patients, assist with forms, and obtain authorizations.
  • Provide excellent customer service, patient advocacy, and culturally sensitive communication.
  • Organize field activities independently and meet enrollment and outreach targets.
  • Notify supervisors of documentation needs and maintain compliance with program protocols.
  • Other duties as necessary


Qualifications

  • Required: High School Diploma/GED (WASC accredited)
  • Required: Bilingual English/Spanish r
  • Required: Reliable transportation, valid driver's license, and current auto-insurance.
  • Highly Preferred: CHW Certificate (HCAI)
  • Preferred: Certified Enrollment Counselor (CEC) preferred or willingness to obtain
  • Minimum 6 months of outreach experience (healthcare setting preferred)
  • Basic computer skills (32 wpm); intermediate preferred
  • Knowledge of health insurance programs (Covered CA, Family PACT, CHDP, etc.)
  • Excellent communication, presentation, and organizational skills
  • Ability to work independently and collaboratively in a fast‑paced environment
  • Willingness to travel (approx. 50%)
