The Lead Nurse is responsible for all activities and operations taking place in the Assisted Living community of a given managed community. Duties include managing care of all residents, providing oversight of the staff regarding activities in the community, communication with medical professionals and families about the needs of residents, maintaining compliance with all regulatory standards and communicating with executive leadership on all appropriate matters. Provides direct supervision of care giving and medication administration within the community. Ensures residents are treated with respect and dignity recognizing individual needs and encouraging independence. Fosters a homelike atmosphere throughout the community.

Responsibilities:

 

  • Conduct and oversee resident assessments, including assessments following changes in condition.
  • Partner with residents, families, and care team members to develop and update individualized service plans.
  • Lead, train, coach, and supervise LPNs, CMAs, Care Partners, and other care staff.
  • Create and maintain staff schedules to ensure appropriate coverage and quality resident care.
  • Monitor medication administration, MARs, documentation, and medication management practices.
  • Review physician orders and assist with transcription and implementation of new orders.
  • Monitor residents' health and psychosocial needs, responding appropriately to changes in condition.
  • Communicate regularly with families regarding resident needs, progress, appointments, and achievements.
  • Coordinate physician, dental, podiatry, psychiatric, laboratory, imaging, ambulance, and other ancillary services.
  • Maintain accurate and compliant resident records and oversee required documentation.
  • Conduct quality, safety, infection-control, medication, and compliance audits.
  • Support the community's Continuous Quality Improvement (CQI) program.
  • Ensure compliance with applicable local, state, and federal regulations, including HIPAA, OSHA, and infection-control requirements.
  • Respond promptly to resident emergencies and participate in on-call responsibilities as required.
  • Assist with inventory management, budgeting, and maintaining appropriate supplies.
  • Participate in family events, community functions, and professional communications.
  • Foster a positive, collaborative environment where residents and staff feel respected, supported, and valued

 

Required Skills and Experience:

  • Valid state nursing license as an RN or LPN/LVN; additional certifications as required by state regulations.
  • High school diploma or equivalent required.
  • Associate or Bachelor's degree in nursing or a related field strongly preferred.
  • Proven leadership and staff management experience.
  • Minimum of three years of assisted living experience preferred.
  • Experience with Microsoft Word, Excel, Windows, and email applications preferred.
  • Genuine compassion for older adults and a strong desire to provide exceptional resident care.

  • Comfortable communicating effectively with residents, families, staff, healthcare professionals, and community partners.
  • Proficient with Microsoft Office and common email application
