Job Overview
Minnich’s Pharmacy is seeking a motivated and compassionate Community Health Worker / Care Coordinator to support and expand our care coordination services. This role plays a vital part in improving patient outcomes through outreach, education, advocacy, and care coordination.

The position works closely with our pharmacy clinical team and provides oversight to pharmacy technician/community health worker team members

Experience

• Required Qualifications
• High School Diploma or GED

Preferred Qualifications

• Bilingual language skills
• Community Health Worker (CHW) Certification
• Experience with Electronic Health Records (EHR)
• Experience working with diverse populations, including low-income and multi-ethnic communities
• Background in case management and/or medical billing

Note: Candidates without CHW certification must obtain certification within 12 months of hire.

• Strong organizational skills with the ability to manage multiple priorities
• Ability to work independently and as part of a team
• Excellent communication and interpersonal skills
• Customer service experience
• Proficiency in Microsoft Office applications
• Ability to remain calm and effective in high-pressure situations
• Dependable and professional demeanor

Pay: $22.72 - $27.36 per hour
