TITLE: Nurse Consultant

REPORTS TO: Regional Director of SNF Operations

Job Summary: The primary purpose of this position is to provide support to Minnewaska Community Health Services (MCHS) and MCHS managed Skilled Nursing Facilities. Provide consultation and completing special projects and long term, interim assignments. Consultation may include operational and clinical assessments, performance improvement, education and training, administrative support, survey readiness, or interim management assignments.

Qualifications: Must have a current/active RN license with the State of Minnesota. Must keep up with CEU's for license renewal. Must have three years RN management skills in a long term care facility. Required to travel within Minnesota for work assignments as necessary. Excellent leadership, communication and customer service orientation. Highly collaborative. Objective and able to work through high pressure situations in a confidential manner. Able to build solid relationships with all levels of the organization including between facilities staff, interdisciplinary teams, residents and families. Knowledge of state and federal regulations. Continuing education of industry trends or other educational opportunities that are pertinent for this position. Flexibility for travel as requested within the state, including over-night travel. Ability to read, write and speak English, and complete and pass a state required background study form which checks for certain criminal convictions or serious/recurring maltreatment.

General Responsibilities:

  • Provides interim nursing home DON management, social service assistance, MDS coordinator guidance as needed.
  • Assists with assessing, planning, developing and implementing systems, processes and other needs as requested.
  • Gather information from residents and their families and have the ability to communicate and comprehend.
  • Educate and train employees on facility policies.
  • Assists with implementation of Quality Assessment and Improvement processes and Process Improvement Projects as requested.
  • Provides technical assistance to facility leadership as it relates to the EHR.
  • Identifies training needs; works with facility managers to plan and conduct training/in-services.
  • Conducts onsite analysis of systems and processes to identify opportunities for improvement; communicates findings to facility management team.
  • Participates in/conducts audits and facility assessments.
  • Participates in regulatory compliance surveys as requested.
  • Assists managers to assure adherence to nursing operations policies/procedures.
  • Maintains current working knowledge of applicable state and federal standards, regulations and industry standards of proactive and professional practice guidelines.
  • Maintains client confidentiality and functions within HIPAA regulations and corporate compliance.
  • Completes special projects assigned by the Regional Director of SNF Operations.
  • Other duties as assigned.
