The ideal candidate has strong clinical judgment, excellent communication skills, and a passion for delivering high-quality, personalized care in the home setting.
Key Responsibilities
- Assess patients' health status and needs in the home setting.
- Develop and implement individualized care plans in collaboration with patients, families, and caregivers.
- Educate patients and families on disease management, health maintenance, and safety.
- Communicate patient progress, concerns, and changes in condition to supervisors and other care team members.
- Maintain accurate and timely documentation of care in compliance with company policies and regulations.
- Promote a safe, comfortable, and supportive environment for patients in their homes.
- Conduct caregiver evaluations and assess caregivers' skills to ensure quality of care.
- Perform supervisory visits with caregivers to observe care delivery, provide guidance, and ensure compliance with care plans.
- Active RN license in the state of practice.
- Previous experience in home health, private duty, or outpatient nursing preferred.
- Strong clinical assessment and patient care skills.
- Excellent communication and interpersonal skills to work effectively with patients, families, and caregivers.
- Ability to work independently and manage a flexible schedule.
- Reliable transportation and a valid driver's license.
- Proficiency with electronic medical records (EMR) and basic computer skills.
- CPR and other relevant certifications as required.
- Willingness to travel up to 75% of the time.
