Key Responsibilities
- Assess patients in their homes and create personalized therapy plans.
- Provide skilled interventions to improve daily living skills and independence.
- Educate patients and caregivers on safety, exercises, and adaptive strategies.
- Document progress and ensure compliance with Medicare Part B regulations.
- Collaborate with the home health/ therapy team to coordinate patient care.
- Adjust treatments based on patient progress and needs.
- Active Occupational Therapy (OT) license in the state of practice.
- Experience in home health or outpatient therapy preferred.
- Knowledge of Medicare Part B documentation and regulations.
- Strong assessment, treatment planning, and patient education skills.
- Excellent communication and interpersonal abilities.
- Reliable transportation and a valid driver's license.
- Comfortable working independently and managing a flexible, home-based schedule.
- Proficiency with electronic medical records (EMR) and basic computer skills.
- CPR and other relevant certifications as required.
