Use
Mind Healing & Wellness Center – Northborough, MA
Mind Healing & Wellness Center is seeking a compassionate, organized, and dependable 
Licensed Practical Nurse (LPN) to join our outpatient mental health and substance use disorder 
team at our Northborough, Massachusetts location.
The LPN will work in close collaboration with our Nurse Practitioners and clinical team to 
support patients receiving psychiatric medication management, substance use disorder treatment, 
Medication-Assisted Treatment (MAT), Spravato, TMS, and other behavioral health services.
This is a collaborative nursing role. The LPN will assist the Nurse Practitioner with medication 
management support, patient monitoring, clinical follow-up, medication ordering and 
coordination, injections, and management of our Spravato, TMS, and MAT caseloads.
Key Responsibilities
• Work closely with Nurse Practitioners in the ongoing care and monitoring of patients.
• Respond to patient medication questions, side effects, changes in symptoms, and other 
clinical concerns, communicating findings to the Nurse Practitioner as appropriate.
• Assist with medication ordering and coordination, including working with pharmacies 
and specialty pharmacies and ensuring medications are available when needed.
• Manage medication refill requests and assist with medication prior authorizations and 
pharmacy-related issues.
• Monitor medications requiring additional follow-up, including laboratory testing, vital 
signs, urine toxicology, and other provider-ordered monitoring.
• Administer and coordinate injectable medications as ordered and within LPN scope of 
practice, including appropriate documentation and follow-up.
• Monitor patients receiving Medication-Assisted Treatment (MAT) and work closely 
with the Nurse Practitioner to ensure required appointments, medication monitoring, 
toxicology testing, injections, and follow-up are completed.
• Assist with coordination and monitoring of the clinic's Spravato and TMS caseloads, 
including scheduling, treatment readiness, patient follow-up, attendance, and treatment 
progression.
• Coordinate ordering and availability of Spravato and other medications required for 
scheduled treatments.
• Maintain and monitor applicable REMS requirements, including required 
documentation, patient monitoring, and program compliance.
• Monitor patients during Spravato treatment and the required observation period, 
including vital signs, response to treatment, and identification of clinical concerns 
requiring Nurse Practitioner involvement.
• Work collaboratively with Nurse Practitioners and TMS staff to monitor patients 
throughout their course of TMS treatment, including attendance, symptom changes, side 
effects, and required follow-up.
• Ensure required provider evaluations and follow-up visits for Spravato, TMS, MAT, and 
medication management are completed.
• Obtain and document vital signs and other appropriate clinical information.
• Assist with urine toxicology collection and monitoring when ordered.
• Follow up on laboratory results, medication orders, injections, and other outstanding 
clinical tasks.
• Coordinate with pharmacies, specialty pharmacies, laboratories, hospitals, treatment 
facilities, and other members of the patient's care team.
• Assist with coordination of care following hospitalization, emergency department visits, 
detoxification, residential treatment, or other transitions of care.
• Respond to patient calls and clinical messages and ensure concerns requiring provider 
evaluation are promptly communicated to the Nurse Practitioner.
• Provide patient education regarding medications, treatment expectations, adherence, and 
provider-established treatment plans within the LPN scope of practice.
• Maintain accurate and timely documentation in the electronic health record.
• Participate in clinical meetings, required training, and quality improvement activities.
The LPN will work in collaboration with the Nurse Practitioner and clinical team. Clinical 
assessment, diagnosis, prescribing decisions, medication changes, and establishment or 
modification of psychiatric treatment plans remain the responsibility of the appropriate treating 
provider.
Qualifications
Required:
• Current and unrestricted Massachusetts LPN license.
• Graduate of an accredited practical nursing program.
• Current BLS/CPR certification.
• Strong clinical communication and organizational skills.
• Ability to recognize changes in a patient's condition and appropriately involve the Nurse 
Practitioner.
• Comfortable working with patients with mental health and substance use disorders.
• Ability to manage multiple clinical responsibilities and maintain appropriate follow through.
• Ability to work collaboratively as part of an interdisciplinary team.
• Comfortable using an electronic health record.
Preferred:
• Psychiatric or behavioral health nursing experience.
• Outpatient mental health experience.
• Substance use disorder or MAT experience.
• Experience administering injectable medications.
• Spravato/esketamine or TMS experience.
• Experience with REMS programs, specialty pharmacies, or medication prior authorizations.
Previous Spravato, TMS, or MAT experience is preferred but is not required for the right 
candidate who is willing to learn.
Who We Are Looking For
We are looking for an LPN who is compassionate, dependable, organized, and comfortable 
working closely with Nurse Practitioners.
The right candidate will take ownership of nursing follow-up, medication coordination, 
injections, MAT monitoring, and Spravato/TMS workflow while maintaining strong 
communication with the Nurse Practitioner. We are looking for someone who follows through,
recognizes when a patient needs additional clinical attention, and makes sure important patient 
needs do not fall through the cracks.
Schedule & Location
Location: Northborough, Massachusetts – In-person
Job Type: Full-Time
The preferred schedule is:
• Two days per week: 10:00 AM – 7:00 PM
• Three days per week: 8:00 AM – 5:00 PM
• Monday through Friday
The specific late days will be established based on clinic and patient needs.
Pay: 35-40dollars an hour.
