We are currently seeking a dedicated and experienced Director of Social Services to lead our team and improve the quality of life for those we serve.
Job Summary
The Director of Social Services is a leadership position responsible for managing the psychosocial, emotional, and social needs of our residents. You will supervise the social services department, develop care plans, lead discharge planning, and ensure compliance with federal, state, and local regulations.
Key Responsibilities
Leadership & Management: Supervise and mentor social services staff, conducting performance reviews and training.
Resident Care Planning: Complete comprehensive psychosocial assessments and develop individualized care plans.
Discharge Planning: Coordinate discharge planning to community and post-acute settings.
Family Liaison: Communicate with families, residents, and the interdisciplinary team regarding psychosocial needs, adjustment issues, and discharge goals.
Regulatory Compliance: Maintain compliance with state/federal regulations (CMS) and company policies, ensuring readiness for surveys.
Grievances: Address and resolve resident/family complaints and grievances.
Requirements & Qualifications
Education: Bachelor’s Degree in Social Work (BSW) or a human services field (sociology, psychology, gerontology) is required.
Licensure: State licensure/certification preferred (LSW, LCSW, or CSW).
Experience: Minimum 1-3 years of experience in a social services leadership role within a skilled nursing facility (SNF) or long-term care setting.
Knowledge: Strong understanding of MDS, care planning, and discharge planning.
