Job Title: LVN – QA / Case Manager (Home Health)
Location: Valencia, CA (In-Office Position)

We’re currently hiring a focused, adaptable, and driven LVN QA / Case Manager to support clinical documentation, quality assurance, and care coordination. This is a great opportunity for someone who is organized, detail-oriented, and passionate about supporting both clinicians and patients.
Responsibilities
Clinical Documentation & Oversight
- Review and monitor clinical notes (SOC, ROC, OASIS, wound care, etc.) for accuracy, completeness, and compliance
- Track timely documentation submissions and manage follow-ups in Devero EMR
- Collaborate with field clinicians to correct or clarify documentation
Quality Assurance & Compliance
- Conduct internal chart audits and maintain QA tracking logs
- Assist with external audit preparation and internal regulatory reviews
- Ensure HIPAA compliance in all documentation and communication
Patient Intake & Provider Coordination
- Assist with new patient intake and setup of episodes of care
- Communicate with clinicians and referral sources regarding missing or incomplete documentation
- Coordinate provider follow-ups and QA review outcomes
Administrative Support
- Scan, upload, and organize documentation in Devero
- Help prepare onboarding materials for new clinicians
- Support inventory tracking and assist with supply coordination
- Provide backup support for admin tasks during peak times or staff absences
Qualifications
- Active California LVN License (Required)
- 1–2 years of experience in healthcare, preferably in home health, wound care, or QA
- Proficiency in Devero EMR and Microsoft Office
- Knowledge of CMS regulations, OASIS documentation, and HIPAA compliance
- Strong organizational, communication, and time-management skills
- Ability to adapt to changing priorities and work both independently and collaboratively
