About Us
We are a compassionate home health agency in Valencia, CA, dedicated to providing high-quality care to patients in their homes. We’re looking for a detail-oriented Home Health Patient Care/ Case Manager to oversee office operations, support clinicians, and ensure quality and compliance.
What You’ll Do
- Manage daily office operations and workflows
- Review and audit clinical documentation (SOC, ROC, OASIS, etc.) for accuracy and compliance
- Coordinate patient care, intake, staffing and follow-ups
- Lead and support clinical and administrative staff
- Maintain HIPAA compliance and assist with audits
- Help onboard new staff and support administrative tasks
What We’re Looking For
- Active California LVN License (preferred) or relevant case management experience
- 1–2 years in healthcare, home health, or QA
- Knowledge of CMS regulations, OASIS documentation, and HIPAA
- Strong organizational, communication, and leadership skills
- Proficiency with Devero EMR and Microsoft Office
- Ability to multitask, work independently, and support a collaborative team

Pay: $27.00 - $30.00 per hour
