Responsibilities and Duties:
As a Weight Management APP with Mill City Medical, you will be responsible for the following key functions:
- Patient Assessment: Conduct thorough assessments of patients' medical history, current health status, and weight management needs.
- Treatment Planning: Develop personalized weight management plans, incorporating a combination of dietary, exercise, behavioral, and medical interventions.
- Patient Education: Educate patients on the importance of healthy lifestyle choices, including nutrition, exercise, and stress management.
- Medication Management: Prescribe and manage medications when appropriate, and closely monitor patients' responses and side effects.
- Collaboration: Work collaboratively with a multidisciplinary MD.
- Patient Follow-up: Monitor patients' progress, make necessary adjustments to treatment plans, and provide ongoing support and motivation.
- Data Collection and Reporting: Maintain accurate patient records and contribute to the evaluation and improvement of weight management programs.
Qualifications:
- Active NP/PA license in Massachusetts
- Board certification in Weight Management preferred
- 2 years of experience in weight management or related field
- Strong communication and interpersonal skills
- Ability to work in a collaborative, team-based healthcare environment
- Commitment to evidence-based practice and continuous learning
