Description
- The Community-Based Programs Service Coordinator provides operational oversight, service coordination and staff leadership across community-based programs for Milestones Services and Milestones RISE.

Responsibilities/Activities
- Ensures client health, safety and welfare (including but not limited to safety, level of supervision, individual needs and behavior training for staff, ensuring appropriate medical care and attention, etc.).
- Ensures client health safety and welfare through identification and reporting of incidents.
- Work with individual departments to coordinate services for Milestones Services and Milestones RISE clients.
- Provide billing oversight for community-based services
- Advocate for clients and promote person-centered services.
- Attend plan of care and team meetings as requested.
- Assist with compliance for new client onboarding, including but not limited to gathering all regulatory documents, meet and greets, behavioral training, team communications, consents, internal document production, etc.
- Assists in maintaining client file compliance and enter information into appropriate databases as necessary.
- Ensures that progress notes and documentation, safety drills and billing documentation for all community-based services meet regulatory compliance, are complete, accurate and submitted in a timely manner.
- Ensures the completeness, accuracy and regulatory compliance of employee documentation and payroll information.
- Assists in reviewing plans of care, prior authorizations and other client documentation for accuracy and completeness.
- Assists with staff and client training.
- Maintains files and documents in an organized format.
- Maintains and adheres to organization policies and procedures.
- Attends community events as needed to develop relationships and market organization programs and services.
- Other duties as assigned.

Schedule
- Full-time Monday though Friday
- On-call rotation as needed
- Will require some weekend and evening hours as needed.
Qualifications/Education
- Associate’s degree in human service field or equivalent leadership experience.
- Has a social security number or valid work permit.
- Current driver’s license.
- Proper Vehicle Insurance Coverage.
Training/Experience:
- Has a minimum of two years of experience working with individuals with serios mental illness and/or with intellectual and developmental disabilities.
- Requires job and organization related training.
