What are the responsibilities and job description for the RN Care Manager - Pediatrics position at MiKids Pediatrics PC?
Schedule:
Open to considering part-time or full-time. This position would also include a weekend rotation doing phone triage, typically once every other month.
Job Description:
Performs care coordination and clinical assessments of pediatric patients with chronic medical conditions, telephone triage, assists physicians with patient messages and needs, and provides patient education
Requirements:
- Demonstrates leadership qualities through excellent time management, work delegation and organization skills
- Demonstrates creative problem solving, critical thinking skills, and excellent written and verbal communication
- Comprehensive knowledge of community resources
- Knowledge of chronic medical and mental health conditions
- Provides care coordination and follow-up for chronic medical and mental health conditions
- Ability to triage and manage complex clinical issues utilizing assessment skills and protocols.
- Ability to work independently as well as in a team environment
- Educates patients and families
- Advocates for patients and family members while maintaining professional boundaries
- Consults with members of the care team on treatment plans and follow-up care
Licenses
- Registered Nurse
- Basic Life Support Certified
Preferred Qualifications
- Care management experience
- 3 years RN experience
