What are the responsibilities and job description for the RN Case Manager position at Midwest Family of Companies and Careers?
CHN is seeking a Part time (20-30 hours per week) RN case manager for Wayne County. Travel to Stark County is possible when on-call.
Case Managers are responsible for completing OASIS assessments at start of care, recertification and completion of supervisory visits. Skilled visits as caseload requires with reasonable productivity requirements.
Manages LPN or RN that assist with skilled visits that CM manages. On call rotation, with compensation allotted for on call week. Would be responsible for some skilled visits and mediation passes.
Mileage reimbursement, company issued iPhone, tablet/laptop for charting, holiday pay, dental, vision, life insurance, and sick time offered. Full time employees eligible for medical insurance and paid vacation time.
