Essential Job Duties & Responsibilities
- Evaluate
patient and perform assessments; clinical documentation; care plan
development; communication with the physician, care team members, patient,
and family.
- Wound
care, Infusion therapy, and Disease Management.
- Provide
direct supervision of nursing clinical care team.
- Administer
medications and treatments prescribed by physician.
- Keeps
medication room and medication cart clean, orderly, and locked according
to regulations. Responsible for security of keys and counting of
medications.
- Make
written and oral reports/recommendations concerning the activities of your
shift as required.
- Ensure
that all aspects of resident care plans are implemented and maintained.
- Admit,
transfer and discharge residents as necessary to meet the resident’s
individual needs and maintain the overall safety of other residents, staff
and visitors.
- Ensure
compliance with infection control standards.
- Participate
in regular case conferences and in-services.

Requirements
- Current Wisconsin nursing license
- Negative TB test within last 6 weeks
- Experience with Point Click Care (PCC)
- Experience in wound care
- At least 3 months experience in long-term care
- Competency to carry out procedures that the physician
orders that are within the scope of nursing
- Proficient communication, organizational and time
management skills
- Able
to use necessary supplies and equipment (feeding pumps, oxygen, treatment
trays, medication cart, etc.).
- Exceptional customer service
- Working as a team mentality
- Reliable
transportation
- Demonstrate
basic computer knowledge and ability with an aptitude to learn company applications,
Experience with PCC a plus.
Compensation
Pay is determined based on years of experience as a Registered
Nurse (RN) or Licensed Practical Nurse (LPN).
Pay Range for LPN: $32-$35 per hour
Pay Range for RN: $53 - $56 per hour
