Behavioral Health Case Manager

Midwest Behavioral Health Center is seeking a compassionate, organized, and resourceful Case Manager to join our multidisciplinary treatment team.

The Case Manager serves as the connection between the care patients receive at MBHC and the people, providers, schools, agencies, and resources that support them outside of treatment. From admission through discharge, the Case Manager helps ensure that communication stays strong, barriers are addressed early, and every patient has a thoughtful plan for what comes next.

The ideal candidate is an exceptional communicator and problem solver who can manage multiple priorities, build strong relationships with families and community partners, and advocate effectively for patients with complex behavioral health needs.

What You'll Do

Coordinate Care & Keep Everyone Connected

• Serve as a primary liaison between MBHC and families, guardians, external providers, schools, state agencies, and community supports.
• Maintain ongoing communication with members of each patient's external support system throughout treatment.
• Coordinate information between the treatment team and external providers to promote continuity of care.
• Build and maintain collaborative relationships with community providers and referral partners.
• Work closely with MBHC's Strategic Partnership Representative and other departments to strengthen connections with community resources.
• Advocate for patients and help ensure access to appropriate services and supports.

Plan for What Comes Next

• Begin individualized discharge planning at admission and continuously update plans as patient needs evolve.
• Develop comprehensive, patient-centered discharge plans designed to support successful transitions to the next level of care.
• Identify potential discharge barriers early and work collaboratively to resolve them.
• Coordinate referrals and connections to outpatient therapy, psychiatric services, educational resources, community programs, and other necessary supports.
• Help patients and families navigate behavioral health systems and community resources.
• Support problem-solving and crisis management when barriers threaten a safe or successful transition from care.

Partner With Patients & Families

• Coordinate family involvement throughout treatment, including scheduling meetings and providing treatment updates as assigned.
• Help patients and families understand available resources and next steps in the treatment process.
• Promote collaborative relationships with families and guardians while maintaining appropriate professional and therapeutic boundaries.
• Ensure patient and family needs, concerns, and barriers are communicated effectively to the treatment team.
• Approach patients and families with compassion, cultural awareness, professionalism, and respect.

Be Part of the Treatment Team

• Participate in multidisciplinary treatment team meetings, case reviews, and treatment planning.
• Collaborate with therapists, nursing staff, medical providers, Behavioral Health Technicians, and other disciplines to support individualized treatment goals.
• Assist with implementation of treatment interventions as assigned.
• Coordinate educational services for patients in accordance with organizational policies and individual treatment needs.
• Participate in therapeutic groups, activities, patient observations, safety checks, and other direct patient care responsibilities as assigned.
• Utilize approved de-escalation and crisis intervention techniques and participate in safety interventions when necessary and consistent with training and organizational policy.

Keep Care Organized & Moving Forward

• Maintain accurate, timely, and compliant case management and coordination-of-care documentation.
• Support utilization management activities as needed.
• Ensure case management activities comply with organizational policies, licensing requirements, accreditation standards, and applicable regulations.
• Monitor assigned goals, performance measures, and key performance indicators.
• Collaborate across departments to identify solutions and improve continuity, quality, and efficiency of patient care.
• Maintain all required competencies, certifications, and training in good standing.

What We're Looking For

Required

• Bachelor's degree in Social Work, Psychology, Counseling, Human Services, or a closely related field.
• Minimum of two (2) years of experience in behavioral healthcare, mental health services, social services, case management, care coordination, or a related setting.
• Knowledge of behavioral health systems, community resources, and care coordination practices.
• Strong verbal and written communication skills.
• Excellent organization, time management, and documentation skills.
• Ability to coordinate multiple cases, priorities, providers, and deadlines in a fast-paced treatment environment.
• Strong problem-solving skills and the ability to identify and address barriers to care.
• Ability to communicate effectively and professionally with patients, families, providers, schools, agencies, referral sources, and multidisciplinary team members.
• Ability to obtain and maintain required crisis prevention, de-escalation, and behavioral management certification.

Preferred

• Experience working with children and adolescents with behavioral health needs.
• Experience in residential, inpatient, psychiatric, or other intensive behavioral healthcare settings.
• Experience with discharge planning and transitions between levels of behavioral healthcare.
• Experience working with substance use disorders and co-occurring mental health and substance use needs.
• Experience coordinating care with schools, community mental health agencies, state agencies, outpatient providers, and other community-based services.
• Familiarity with utilization management, payer requirements, or authorization processes.
