Overview
Provides case management services according to guidelines established by the Agency for Health Care Administration; Florida Medicaid, Home and Community Based Waivers Unit, and the Florida Department of Health, Bureau of HIV/AIDS. Maintains client confidentiality according to the agency’s policies and procedures. Reports to an assigned case management supervisor.
Responsibilities:
- Comprehensive Needs Assessments and identify medical, functional and social service needs of the recipient.
- Advocate for the needs of the patient.
- Coordinate the completion of Medical Needs Assessment on all new patients.
- Periodic face-to-face reassessments.
- Develop and update plans of care based on the recipient’s acuity level in consultation with the recipient, caregiver, the physician’s office, Midway staff, and other relevant parties.
- Provide updated copies of the plan of care to the recipient as requested.
- Coordinate efforts to access services provided by all funding sources including Medicaid and non-Medicaid sources.
- Follow up with the recipient and the provider to monitor the quality, quantity, duration and effectiveness of the services provided.
- Maintain documentation for all services authorized and any follow up activity.
- Maintain required face-to-face and telephone contacts with the recipient as described in the client’s “level of need”.
- Maintain updated, signed, dated and legible case notes that document the changing needs and welfare of the recipient.
- Protect the privacy and confidentiality of the recipient.
Education and/or Experience
- Bachelor’s degree in a social service or health field.
- Bachelor’s degree in a field other that social services or health and have a minimum of one year of case management experience.
- Registered Nurse without a Bachelor’s degree who has a minimum of one year case management experience.
- Licensed Practical Nurse without Bachelor’s degree who has a minimum of three years case management experience
- Have four years of verifiable experience case managing individuals with HIV/AIDS at an established agency that can substitute on a year-for-year basis for a bachelor’s degree.

Pay: $45,000.00 - $52,500.00 per year
