What are the responsibilities and job description for the Caregiver-Plover, WI position at Midstate Independent Living Choices, Inc (MILC)?
Respite Provider needed in Plover, WI.
Up to 15 hours/week. Tuesday and Thursday afternoon/evenings (maybe 1pm-6pm) some flexibility. Ability to transport required.

RESPITE WORKER/SUPPORTIVE HOME CARE WORKER provides confidential services to physically or developmentally disabled individuals and/or frail elderly consumers in their home.
DUTIES: Respite Worker/Supportive Home Care Workers will be trained in the provision of services and in each skill that is assigned. Completion of all time sheets and documentation necessary to maintain program services in a thorough, accurate, and timely manner. Maintains confidentiality of consumer's information. Adheres to and abides by all agency policies and procedures.
QUALIFICATIONS:
Familiar with the philosophy and concepts of independent living required
Ability to understand and carry out consumer instructions required
Ability to relate well to others required
Good communication skills required
Good past employment references required
Ability to: regularly sit, stand, and walk; use hands and fingers, handle, and feel; reach with hands and arms; bend; squat; twist and talk or hear; occasionally lift and/or move individuals using transferring techniques or assistive technology/equipment. Completion of interview and training conducted by PAS staff required
Effective means of transportation to the work site required.
If transporting is part of plan, auto insurance must meet agency requirements and must meet driving standards.
Satisfactory Criminal Background results required. Results based on relevance to position and in compliance with state and federal regulations for personal care workers.
