Hospice RN Registered Nurse (RN) – Case Manager
Location: Kansas & Missouri service areas (travel required)
Schedule: Full-time; some on-call participation per rotation
Pay Transparency: The salary range for this position is $75,000 – $90,000, depending on experience, education, and internal equity. While we are proud to share our pay ranges openly, it’s important to note that we do not always hire at the top of the range. Offers are determined based on several factors, including relevant experience, scope of responsibilities, and alignment with organizational pay practices.
About the role
We’re seeking a compassionate Registered Nurse to plan, deliver, and coordinate patient-centered care in homes and community settings. You’ll use the full nursing process assessment, planning, intervention, and evaluation while educating and supporting patients and families to achieve optimal comfort and independence.
What you’ll do

Complete initial and ongoing comprehensive assessments and histories.

Write, initiate, and regularly revise individualized plans of care.

Determine nursing diagnoses and implement preventive/rehabilitative procedures.

Administer medications and treatments as prescribed; provide clear patient/family teaching.

Support activities of daily living and serve as Case Manager when assigned.

Document timely, accurate clinical notes; maintain a safe care environment.

Collaborate with physicians, caregivers, and the interdisciplinary team; attend team meetings.

Participate in on-call duties as defined by management.
Qualifications
What you’ll bring

Current RN licensure in Kansas and Missouri and CPR certification.

1–2 years recent acute/institutional experience and 1–2 years recent home-care experience.

Valid driver’s license with insured, reliable vehicle.

Strong assessment, documentation, communication, and problem-solving skills.

Ability to stand/walk for prolonged periods; reposition/lift patients and equipment.

Bachelor’s degree preferred; supervision of nursing aides may be required.
