Midland Care Connection is a not-for-profit, community-based organization in Topeka, Kansas, dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas.
Home Health Program
Provides short-term, skilled medical care to individuals in their homes to support recovery, manage chronic conditions, and help patients remain as independent as possible. Services are delivered by a coordinated team of healthcare professionals and may include nursing care, therapy, and education to help patients and families manage health needs safely at home. The program works closely with physicians and caregivers to create personalized care plans that promote healing, prevent hospital readmissions, and improve overall quality of life.
RN Case Manager
Coordinates and oversees patient care to ensure safe, effective, and personalized services in the home. They assess patient needs, develop and update individualized care plans, and collaborate closely with physicians, therapists, and other care team members. The RN Case Manager also provides skilled nursing care, monitors patient progress, educates patients and families, and serves as the main point of contact to ensure continuity of care and positive health outcomes.
Qualifications:Required
- Current, unrestricted Registered Nurse (RN) license
- Graduation from an accredited nursing program
- Experience in home health, case management, or community-based nursing
- Strong clinical assessment and care coordination skills
- Ability to develop, implement, and update individualized care plans
- Knowledge of Medicare, Medicaid, and home health regulations
- Excellent communication and documentation skills
- Ability to work independently and manage time effectively
- Valid driver’s license with reliable transportation
Preferred
- Bachelor of Science in Nursing (BSN)
- Previous RN Case Manager experience
- Experience with electronic health records (EHR)
- Familiarity with interdisciplinary team-based care
- Strong problem-solving and patient education skills
