Midland Care Connection is a not-for-profit, community-based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, it offers a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and its signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas.
PACE Program
The PACE (Program of All-Inclusive Care for the Elderly) program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity.
Community LPN
Provides hands-on nursing care to older adults in their homes and community settings, helping them remain safe, healthy, and independent. Working under the direction of an RN and as part of an interdisciplinary care team, the Community LPN monitors participants’ health, reports changes in condition, assists with implementing care plans, supports care coordination, and builds strong relationships with participants and families. This role combines clinical skills, independence, and teamwork to deliver participant-centered care outside of a traditional facility setting.
Qualifications:Required
- Current Licensed Practical Nurse (LPN) license in the state of Kansas
- Graduate of an accredited practical nursing program
- Ability to provide nursing care under the direction of an RN and within scope of practice
- Experience or comfort working independently in home and community settings
- Strong observation, documentation, and communication skills
- Ability to collaborate effectively with an interdisciplinary care team
- Valid driver’s license and reliable transportation for community visits
- Basic computer skills for electronic health record documentation
Preferred
- Experience working with older adults, chronic conditions, or geriatric populations
- Previous experience in home health, community health, PACE, or long-term care
- Familiarity with care coordination or interdisciplinary care models
- Knowledge of PACE philosophy or person-centered care approaches
- Strong relationship-building skills with participants and families
