Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Community Care Hospice
Provides compassionate end-of-life care for individuals with serious, life-limiting illnesses, focusing on comfort, dignity, and quality of life rather than curative treatment. Through an interdisciplinary team that may include nurses, social workers, chaplains, and other specialists, they offer pain and symptom management, emotional and spiritual support, and guidance for patients and their families. Care is typically delivered wherever the patient lives—such as at home or in a senior living community—and emphasizes personalized support for both patients and loved ones throughout the end-of-life journey.
Hospice Social Worker
Supports patients and their families by addressing emotional, social, and practical needs during end-of-life care. They provide counseling, help families cope with changes and grief, assist with advance care planning and decision-making, connect patients to community resources, and offer ongoing support to ensure both patients and loved ones feel informed, supported, and cared for throughout the hospice journey.
Qualifications:Required
- Bachelor’s or Master’s degree in Social Work (BSW or MSW)
- Current state social work license (as required by state regulations)
- Experience working with patients and families in healthcare or human services
- Knowledge of psychosocial, emotional, and end-of-life needs
- Strong communication and documentation skills
- Ability to work as part of an interdisciplinary care team
- Valid driver’s license and ability to travel to patient locations
Preferred
- Master of Social Work (MSW)
- Previous hospice, palliative care, or healthcare experience
- Experience supporting patients and families during grief and loss
- Knowledge of community resources and referral processes
- Comfort providing emotional support during end-of-life care
- Experience with electronic medical records (EMR) systems
