Established in 1978, it offers a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and its signature PACE (Program of All-Inclusive Care for the Elderly) program. PACE Program
The PACE program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity.
Certified Nursing Assistant
A PACE Center CNA helps support older adults who are enrolled in the Program of All-Inclusive Care for the Elderly. Their work typically includes assisting participants with daily activities such as bathing, dressing, grooming, and mobility, as well as monitoring vital signs and reporting any health changes to the nursing staff. They may also help with meal assistance, accompany participants during activities, and provide overall support to ensure comfort, safety, and independence in both the center and participants’ homes.

Qualifications
Required: Current Home Health Aide (HHA) certification or Certified Nurse Aide (CNA) certification with willingness to enroll in HHA certification course within first 90 days of employment. Preferred: One year experience as a Home Health Aide preferred.
