Midland Area Agency on Aging is one of 667 Area Agencies on Aging nationwide and one of eight in Nebraska that help ensure older Nebraskans remain independent, healthy and safe in their homes and communities.

Midland Area Agency on Aging is looking for a full-time Care Manager. This position will be working in our Case Management Department.

Primary Responsibilities:

· Perform screening & assessment of clients.

· Work with the general public, particularly adults who are elderly or disabled

· Support consumer direction and decisions

· Knowledge about community resources and educate people about their options

· Assist in connection individuals to programs, services and public benefits

· Identify long term services and supports

· Provide follow up and monitoring of client visits.

· Help in individuals to remain safely their home.

Required Qualifications:

- Associates Degree

-At least an equivalency of two years of related, professional experience; paid or unpaid; (employment; college internships; volunteering at philanthropic, community and social organizations) in long-term care, gerontology or community health.

Candidates will receive credit for all qualified experience.

- Bilingual (fluent) in English and Spanish preferred.

· Current Driver’s License

· Experience working with MS Word, Excel, and Outlook

· Ability to travel in assigned region to visit members in their homes

· Case Management experience

· Behavioral health experience

· Field based work experience

· Experience in long-term care, home health, hospice, public health, or assisted living

· Knowledge of community resources

Salary: Starting $22.00- $23.00 per hour

Schedule:

8 hour shift

Monday to Friday

• Identify long term services and supports
• Authorize services, collect, record and data.
• Provide follow up and monitoring of client visits.
• Help in individuals to remain safely their home.

Required Qualifications:

• Registered Nurse with current Licensure or
• Bachelor’s degree in Social work or related field.
• At least 2 years of experience in long-term care gerontology and/or care coordination is required
• Experience working with MS Word, Excel, and Outlook
• Ability to travel in assigned region to visit members in their homes

Preferred Qualifications:

• Case Management and/or Care Coordination experience
• Behavioral health experience
• Field based work experience
• Experience with electronic charting
• Experience in long-term care, home health, hospice, public health, or assisted living.
• Background in managing populations with complex medical or behavioral needs
• Knowledge of community resources
