Position Highlights
Department: Center for Chronic Care Management
Hours: Part-Time/20 hours
Shift: Days, 1st Shift, no weekends or holidays
Position Summary:
The LCSW Care Manager is responsible for focusing on providing support in the management of anxiety and depression for patients dealing with chronic disease as well as support and education for patients who wish to quit tobacco addiction.
Minimum Qualifications:
Licensed Clinical Social Worker (LCSW) able to practice in the State of Connecticut.
Minimum of 5 years of experience in acute care, home care plus experience in outpatient settings
Willing to obtain Certification as Tobacco Treatment Specialist within 1 year of hire
Preferred Qualifications:
Certification as a Tobacco Treatment Specialist (CTTS) is preferred
Care Management or Care Coordination experience is desired
